Description: Lorain County Public Health is seeking to hire a part-time, contract-based Neighborhood Navigator currently residing in Lorain (zip codes 44055, 44052) or Elyria (zip code 44035). Primary duties will be within the Community Health Division and Health Promotion Chronic Disease Prevention Divisions. This is a part-time (20 hours/5 days per week) position with usual hours between 8:00 am and 4:30 pm Monday through Friday. This position will require some weekend and evening hours to attend community events and to meet the needs of clients. Schedule may vary week to week and will be arranged and modified directly with supervisors and team members.
Under the general supervision of the Director of Health Promotion & Chronic Disease Prevention, the Neighborhood Navigator is responsible for identifying and connecting women, with a focus on African American women, to existing services through a short-term relationship of screening, referral, and follow-up. The goal is to increase support for pregnant women and their families, to improve birth and health outcomes. This position is responsible for the planning and execution of deliverables associated with various grant requirements.
MAJOR WORK CHARACTERISTICS
Knowledge of: basic computer skills including email, Internet, Microsoft Office, and Google Suite.
Ability to: represent all program partners with integrity and respect, keyboard with accuracy and speed, manage time effectively, and function on a flexible schedule.
Must have a valid Ohio driver’s license with ongoing proof of auto insurance and be able to travel with reliable transportation.
PHYSICAL REQUIREMENTS & UNUSUAL WORKING CONDITIONS
Must be able to sit and stand for long periods as well as bend and stretch to access various physical filing systems. Be able to lift objects of weight up to 25 pounds.
COMPENSATION: $15.00-$22.00/hour This is a contract position. No employment or benefits are assigned.
